Phase IV of Bariatric Surgery Post-Op Diet

Soft foods also known as the adaptive stage, it’s during this phase that may start to eat foods that can be easily mashed with a fork or a spoon as you continue to observe a nutrient goal of 60-70 grams of protein from high-quality lean sources, 64 ounces of fluids every day, up to three servings of soft vegetables, and a fat source from a single serving of ripe avocado.  

  • Lean chicken breast, lean turkey breast, all varieties of fish, soups diluted with skim milk or add finely minced or diced meat, egg whites, hard boiled or poached eggs, non-fat cottage cheese, non-fat cheese, low fat deli meats and soft tofu are some of the recommended sources of protein
  • Avocado, squash, baked potatoes, sweet potatoes, yams, green beans, tomatoes, squash, cucumbers, and carrots are great soft food vegetable options at this stage of the diet
  • Canned fruits, cooked or canned vegetables, soft, fresh fruit such as apples, pears, and peaches without skins, bananas will be your fruit options
  • Grains such as hot cereals, boiled pasta, noodles, white rice, unsweetened cereals with skim milk are the recommended sources of starches
  • Water, decaffeinated coffee, tea, broth, sugar free fruit juices and other beverages with artificial sweeteners are liquids that you should keep handy.

    A typical Phase IV meal plan may include the following foods:

    Breakfast – 4 scrambled eggs or egg substitute, ¼ cup hot cereal, ¼ cup canned peaches in own juice, a multivitamin and B12 supplement

    10:00 am – ¼ cup of non-fat cottage cheese

    11:00 am – skim milk and calcium and vitamin D vitamin dose

    Lunch – ¼ cup of lean turkey breast or tuna, ½ cup carrots or ¼ cup of cooked green beans, 8oz of vegetable juice (to be taken after the meal)

    2:00 pm – broth and multivitamin

    3:00 pm – 2 Oz of lean chicken and a tablespoon of avocado for fat

    4:00 pm – 8 Oz of skim milk/ water and calcium supplement

    5:00 pm – ½ cup of either squash, baked potatoes, sweet potatoes, or yam

    6:00 pm – 8 Oz of water

    7:00 pm – 8 Oz of skim milk plus calcium and vitamin D dose

    8:00 pm – ¼ cup white fish, ¼ cup sweet potato, ¼ cup cooked green beans, 8 Oz water

    Phase IV Diet Reminders
  • Consume three small meals each day and eat the protein portion of your meals firsts
  • And space out your liquids between meals about 30 minutes after each meal
  • Sip all fluids slowly to prevent nausea and vomiting. Protein continues to be a key concern at this stage, therefore, remember to consume an 8 ounce protein supplement or shake, followed by 16 ounces of sugar free, clear liquids
  • Carry a water bottle with you all the time to maintain hydration and spread your fluid intake over a three-hour period
  • You will still need to take two multivitamins, two to three 400-600 mg doses of calcium citrate, 1,000 IU of Vitamin D3 per day, which may be divided into two 500IU doses to be taken together with calcium citrate. Your doctor may also recommend that you take vitamin D3 and a daily dose of 500-1,000 mcg sublingual vitamin B12 or a monthly
  • Avoid skins and seeds of fruits and vegetablesas well as bread
  • Chew all your foods thoroughly and stop eating at any time you feel full
  • Compliance with these recommendations cannot be overstressed in order to avoid negative consequences such as discomfort, medical complications, and nutrition deficiencies.
