Why revise a sleeve?
This can happen if the sleeve has stretched over time, often with increased food intake, or because of a complication.
Regaining weight after losing it as expected may mean the stomach has stretched. Can your stomach stretch?
Chronic acid reflux, vomiting or nausea that medication and diet changes don’t relieve.
Revision isn’t always surgery. Nutrition support, GLP-1 medication and endoscopic treatment are also options, as the ASMBS statement on weight recurrence explains.
Your options
Our surgeons recommend a revision after reviewing why your sleeve isn’t working, your reflux and your records. All sleeve revisions are custom quotes.
| What it does | Often considered for | At Renew | |
|---|---|---|---|
| Sleeve to gastric bypass | A small pouch is made from the sleeve and connected to the small intestine, so food bypasses part of it | Reflux, and weight that wasn’t lost or came back | Custom quote |
| Sleeve to mini gastric bypass | The sleeve is connected to a loop of small intestine with one connection | Weight that wasn’t lost or came back | Custom quote |
| Sleeve to SADI-S | The sleeve stays, and the duodenum is connected to the lower small intestine | More weight loss and type 2 diabetes | Dr. Pérez, Tijuana and Cancún. Custom quote |
| Re-sleeve | The stretched sleeve is made narrow again, much like the first surgery | A sleeve that has stretched or widened | Custom quote |

What the research says
Often the choice when reflux is the main problem. In the SLEEVEPASS trial, reflux was the most common reason sleeve patients needed another operation (SLEEVEPASS).
A 2023 systematic review found SADI led to more weight loss than gastric bypass for up to 5 years after revision (Obesity Surgery).
In a study of 79 re-sleeve patients followed for about 3.7 years, BMI fell from 40.0 to 33.1. Complications occurred in 10.1%, and no fistulas were found (El Khoury et al.).
One connection instead of two. In a trial of first-time surgery, reflux was more common after mini bypass than after Roux-en-Y (YOMEGA).
Revision surgery has more risk than a first operation because your surgeon works on tissue that has already been operated on.

“Absolutely loved my time here. I had previously had a sleeve performed in the states, but came here for the bypass Revision and so glad I did. All the staff were super friendly and the nurses and doctors answered all questions I had during the procedure.”
